Instructions for Completion of Assessment Record Book Forms for Instructional Programs/ Administrative Units

Instructions for Completing Form C:

  • You will have one Form C for each Intended Educational (Student) Outcome or Administrative (Educational/Service) Objective stated on Form B. Thus, if there are three Objectives/ Outcomes listed on the Form B, there will be three Form Cs.

 

  • Provide the name of the instructional degree program and degree level or administrative department in the box provided below.  Indicate the period of time covered by this form.

  • In the box below identified as Intended Educational (Student) or Departmental Administrative (Educational/Service) Objective, transfer the first outcome from Form B.  On the second Form C, transfer the second Intended Educational (Educational) Outcome or Departmental Administrative (Educational/Service) Objective from Form B, and continue this process for all outcomes.

  •  

  • Note: There should be one form C for each intended objective/outcome listed on form B.

  • Complete the boxes under the "First Means of Assessment for Outcome Identified Above" subsection according to the directions listed below:

  • In the box below identified as "Means of Program Assessment & Criteria for Success, identify  the source of your assessment information (e.g., practicum supervisor's report, senior projects, graduating student survey, or Major Field Achievement test) used to determine the accomplishment of the intended educational (student) outcome or administrative (educational/service) objective listed in the top box.

  •  

  • In addition to describing the means of assessment, also include a statement of the criteria or benchmark (target) for judging the department's or program's success in meeting this intended outcome or objective.  The criteria for success should answer the question: If our instructional program is functioning the way we think it 'ought' to function, what will be our students' achievement on this means of assessment (usually expressed in terms of an average score or percent or percentile).
