  Instructions for Form B

  • Each Instructional Degree Program and Administrative Department  will have a Form B and one Form C for each of their Intended Educational (Student) Outcomes or Administrative (Educational/Service) Objectives listed below.

  •  

  • Instructional (academic) departments should complete the four boxes below identified as "Instructional Degree Program," "Degree Level," "Assessment Period Covered," and "Date Submitted" with the identical information as provided on Form A previously completed. 

  •  

  • Administrative/educational support departments should list only the name of the department as previously indicated on Form A and indicate the period covered by this form and date submitted.
