SPORTS MEDICINE

FRESHMAN AND OTHER FIRST-TIME PARTICIPANTS MUST COMPLETE:

1. The Health History Form. Please be as complete and accurate as possible.
   
2. Pre-Participation Physical Form. Filled out by your physician.
   
3. Risk Waiver Form.
   
4. Complete the Athlete Insurance Questionare Form  and enclosed a copy (front & back) of the identification card of any insurance policy that you are covered under. The College, like most other institutions carries a secondary athletic accident insurance policy on all of our student-athletes. Secondary coverage means that any bill incurred as a result of athletic participation must first be submitted to any other insurance that the athlete is covered by and our policy will pay any remaining costs. Please make sure that this form is filled out completely.

ALL RETURNING STUDENT-ATHLETES MUST COMPLETE:

FAILURE TO COMPLETE ALL OF THE APPROPRIATE FORMS BEFORE THE FIRST DAY OF PRACTICE WILL PRECLUDE YOU FROM PARTICIPATION IN ATHLETICS AT RIVIER COLLEGE UNTIL THEY ARE COMPLETED.
